48-hours notice is required to cancel or reschedule. Please be advised you will be charged the full fitting fee if you cancel within this time frame or do not show.

APPOINTMENT NOTES

Please check in at least 5 minutes prior to your appointment. Please bring a pair of indoor shoes or clean soled golf shoes. If you wear a golf glove please remember to bring one.
